Passport Services | Summit County Clerk’s Office, Utah

Passport services are by appointment only. Appointments are available on the hour between 9 a.m. and 4 p.m. Monday through Friday. 

Application

To get a new passport, you must apply in person at the Summit County Clerk's Office. 

Please bring with you: 

  • A passport photo. Photos can be taken at the Summit County Clerk's Office for $10). 
  • Government-issued photo identification. 
  • A certified copy of your birth certificate. 
  • Previous passport (if applicable). 
  • Completed passport application. Please make sure to print it single-sided. You can also obtain and fill out an application at the Summit County Clerk's Office. 
  • Two forms of payment, one of which has to be a check or money order, the other can be cash, check, or credit card.

Minors

All children under age 16 must apply for a passport in person with two parents or legal guardians. You must bring the child's original/certified birth certificate and a government-issued photo identification for each parent. 

One parent may accompany the child if they have a notarized statement of authorization and a copy of their driver's license (front and back). Parents with sole custody may also appear with the child and submit primary evidence of sole authority to apply. 

Minors who are 16 to 17 years old must be accompanied by 1 parent with government-issued identification.
