Voter Registration Forms 

Please complete the appropriate form, sign with a physical signature and return it by email, mail, or in-person to the Summit County Clerk.

Party Affiliation Change Form 

Please return a completed Party Affiliation Change Form and return it by email, mail, or in-person to the Summit County Clerk's Office. 

Note: at certain times state law suspends party affiliations until after partisan primary elections. Utah Code section 20A-2-107. At those times the Office will make the requested change following the primary election.

Ballot Request Form 

Political parties will hold their Primary Elections on June 23, 2026. You will receive a ballot for the party you are registered with. If you want a Democratic Ballot but are not registered as a Democrat, you must request one. Please return a completed Ballot Request Form by email, mail, or in-person to the Summit County Clerk. The deadline to request a ballot and receive a ballot in the mail is 5:00 pm on June 12, 2026.

Each voter may submit only one ballot per election. Submitting more than one is prohibited by law.

  1. Voter Registration Information

Register to vote, update your voter registration, or check to see if you are registered to vote at vote.utah.gov.

The deadline to register and receive a vote by mail ballot for the Primary Elections is June 12, 2026. Voters who are not registered, but are otherwise eligible to vote, may vote provisionally in person during Early Voting or on Election Day.

Requirements to Register to Vote:

  • Must be a U.S. Citizen
  • Must be a resident of Utah for at least 30 days prior to election
  • Must be 18 years of age on or before the General Election
  • Must reside at the address where you are registered

Note: If you are 16 or 17 years old, you can pre-register to vote. If you pre-register, you will automatically be registered to vote when you turn 18 years old.

  1. How do I vote?

Active registered voters will receive a ballot in the mail about three weeks before the election. There are several ways to return your ballot. 

  • By Mail: General Election ballots must be received by the election office by 8:00 pm on General Election Day November 4, 2025. If you do not wish to receive a ballot by mail, please send an email stating that to elections@summitcountyutah.gov
  • Dropbox: General Election ballots must be deposited in an official dropbox by 8:00 pm on General Election Day, November 4, 2025.
  • In Person: Visit a Voting Assistance Center during Early Voting or on Election Day if you did not receive your ballot in the mail or you need assistance with voting.

How does vote by mail work?

Your county clerk uses various security and privacy measures to ensure that your mail ballot is secret and counted accurately. Watch this short video that explains their process:

Do I have to mail my ballot?

No. If you do not want to mail your ballot using the United States Postal Service, you have other options to turn in your ballot:

  • Drop your ballot off at a drop box location.
  • Drop your ballot off at an early voting location or an Election Day voting location. Find a location near you.

You can return your ballot by mail but it must be received by the election office by 8:00 pm on Election day. Please be sure to allow enough time for your mail to arrive by the deadline. If you prefer to use a secure drop box, your ballot must be dropped off before 8:00 pm on Election Day.

What happens if the signature on my ballot envelope doesn't match?

Your county election officials will compare the signature on your ballot envelope with the signature on your previous ballots and your voter registration form. If your signature does not match, your county clerk will notify you. You will be given an opportunity to submit an affidavit called a “cure letter” that allows you to affirm that your signature is correct. Once your county election official receives your affidavit, the ballot will be counted.

  1. What if I am away from home during the election?

Elections in Summit County are conducted mainly by mail. Ballots are mailed to active, registered voters 21 days prior to an election according to Utah state law.

The post office does not forward ballots. If you will be out of town for the entire period between the date ballots are mailed and Election Day and need your ballot mailed to an address in the U.S. other than the address where you are registered to vote, please submit an Alternate Address Request Form

If you will be out of the country, please submit a Federal Post Card Application Form.

To receive a mail-in ballot you must be registered to vote no later than eleven (11) days before an election. Please contact the Clerk's Office with any questions 435-336-3040.

  1. Federal Voting Assistance Program

The state offers an electronic ballot delivery method provided through the Enhanced Voting platform. Electronic voting instructions will be emailed approximately 45 days before Election Day and electronic ballots must be received by the Elections Office no later than 8:00 pm (Mountain Standard Time) on Election Day.

Federal Voting Assistance Program

Military and overseas voters can contact the Federal Voting Assistance Program (FVAP), which provides tools and resources to ensure that U.S. citizens who want to vote can do so from anywhere in the world.

Voters can find instructions and the Federal Post Card Application (FPCA), which is the registration form, or the Federal Write-In Absentee Ballot (FWAB), the backup ballot, online.
